In a QI project, the 5W2H tool helps teams identify key information needed to fully understand their QI issue and draft a problem/opportunity statement.
In Step 1 of this template, answer the ‘5Ws’(Who? What? Where? When? Why?) and the ‘2Hs’ (How? and How often?). In Step 2, use your answers to draft a problem/opportunity statement for your QI project.
Remember, a well-defined problem is already half-solved. Your problem/opportunity statement may go through multiple revisions before it feels just right!

Problem/Opportunity Statement: A clear explanation of the issue or opportunity that needs attention. It defines the problem you’re trying to solve or the opportunity for improvement in a way that helps everyone involved understand the goal. It’s a starting point for planning changes that will lead to better outcomes.
