CMHA Toronto-CMHA Toronto strengthens discharge planning for better continuity of care

Project status as of June 12th, 2024


While many clients at CMHA Toronto have positive transition experiences, the organization noticed inconsistencies in how discharge plans, across its programs, were communicated and delivered. Satisfaction surveys revealed that not all clients experienced the same level of clarity and preparation during this critical transition. To address this, the team initiated a quality improvement project to standardize discharge practices across programs. Key steps include clarifying staff expectations, introducing consistent protocols, , and embedding client feedback mechanisms into the transition planning process. These improvements are designed to ensure every client exits care with a comprehensive, well-communicated plan that reflects their ongoing needs and supports their next steps.

Problem/Opportunity Statement:

Discharge experiences vary across programs, with gaps in how clearly and consistently transition plans are communicated. As an organization, we want all transitions to be carried out in a consistent manner and address client needs to support a successful transition in care.


AIM Statement:

By March 2026: increase the percentage of clients who report having a clear transition plan that met their needs from 76 per cent to 85 per cent.
