Project status as of May 13th, 2025
When a team asked, “How can we make care planning more effective for residents and simpler for staff?”, they launched a quality improvement project to find out. By exploring the root causes of the issue, the team discovered that monthly goal setting wasn’t being used to its full potential. It often became difficult to follow through consistently, which made it harder for staff and residents to stay engaged and see progress. With coaching and structured testing, they introduced targeted solutions that helped realign care plans and goal setting to what matters most – meaningful outcomes and shared accountability.

Problem/Opportunity Statement:
The current approach to supporting individuals in meeting their monthly goal-setting, has not been used to its full capacity and gets lost in execution. This leads to lack of motivation, creating barriers and no measured outcomes. This disconnect leaves the staff and residents feeling frustrated, as though they are completing chores rather than engaging meaningfully.
AIM Statement:
To improve or maintain resident satisfaction with care planning and goal setting between 4.01-4.5) and increase staff satisfaction from 3.15 to 4.01-4.5 by April 2025.
