CMHA North Bay & District – Creating an Organizational Foundation for the use of the OCAN

Project status as of June 12th, 2024


This team sought to promote the use of OCAN during the care-planning process for clients after noticing that more than half of their Intensive Case Management staff were not using the OCAN to guide their clients’ care plans. Based on staff feedback, they realized that it would be helpful to provide staff with training on how to use the OCAN most effectively. For their project, they developed a series of online modules on the role of OCAN in care-planning and delivered it to their Case Management team. All of the staff that completed the course reported that it was helpful for them in their role and that they would recommend it as training to their peers. The team continues to monitor the use of OCAN among case management staff.

Problem/Opportunity Statement:

Since January 2020, CMHA NBD’s Intensive Case Management program has not been using the OCAN as a useful tool or its information in a meaningful way. This negatively impacts clients, staff and management’s engagement with the OCAN and its potential use to highlight how CMHA NBD services are supporting clients.


AIM Statement:

Increase the percentage of clients with high agreement for the question “Staff and I agreed on my treatment services and support Plan” in the Ontario Perception of Care Tool from 0% to 20% by December 2024
