When medication errors began rising in CMHA Kenora’s safe bed and supportive housing programs, the team knew they had to act quickly. By digging into their data and exploring root causes, the team created a plan to address the most critical contributors to the problem. The strategy included strengthening staff orientation, enhancing post-incident follow-up and phasing in an electronic medication record (MAR) system synchronized with existing client management software. Through testing, staff feedback and a strategy to address sustainability challenges, the results are already showing. By April 2025, medication errors dropped to zero, and the e-MAR is now fully implemented across both programs.

CMHA Kenora has seen an increase in medication errors. Between March 2024 to March 2025, there were an average of four medication errors per month combined in the Kenora Supportive Housing and Safe Bed programs.
AIM Statement:
By May 31, 2025, reduce the average number of monthly medication error incidents from four to two in the residential programs.
