Project status as of June 12th, 2024
This team sought to improve their data collection procedures, especially those related to service user discharge. As the project progressed, the team realized that some of the data errors in their discharge records were caused by premature discharging of service-users. To reduce the rate of premature discharges, the team provided additional training to staff on the use of OCAN for client care and discharge planning. This training has allowed them to clarify the discharge process for staff and clients and ensure that clients are consistently meeting their care plan goals.

Since 2020, we have observed that 30% of our service- users are discharged before they achieve their care plan goals.
AIM Statement:
By June our aim is to increase the rate of service user care plan goal by 10%; and by December another 10%; and by March 2025 5% (overall 25% increase).
